This page includes a resource guide, printable documents and questionnaires that can help families prepare in case someone goes missing.
Be prepared
Health, cognition and personal information can change over time. Review and update completed questionnaires at least once a year to make sure the information remains current.
Keep printed copies in an easy-to-find location, such as on top of the fridge, so they can be quickly accessed in an emergency.
