Cochrane Connect Card

The Cochrane Connect Card helps you save money for the things that matter most. The card provides low-income households access to services and programs with a discount.

Eligibility for the Cochrane Connect Card program is based on financial and situational assessment. See the following table for household income eligibility: 

To apply for the program, you must book an appointment with a resource worker and provide the following documentation for all adults in the household.

Financial information: 

• Canada Revenue Agency Notice of Assessment (with line 15000), or 
• AB Works, EI or AISH confirmation (income support) 

Proof of address: 

• Mortgage or lease agreement, or 
• Current bill (showing name and address, dated within three months) 

Apply now: 

• Families with children under the age of 18, book an appointment with Family Resource Network (see contact information below)
• Individuals or families with children 18+, older adults or rural residents, book an appointment with Family and community Support Services (see contact information below).
